I represent that I have the
authority to authorize the herein described work and hereby expressly
authorize this work to be done and to determine the cause of and to correct
the trouble described along with the necessary labor, parts, material and
outside services necessary. I hereby
grant you and/ or your employee’s permission to operate any equipment and
taxi the aircraft herein described for the purpose of testing and / or
inspection. I further agree to pay
costs incurred on aborted repairs and in cases where a specific problem
cannot be found and / or duplicated.
I acknowledge that your warranty shall cover only the specific items
installed, parts replaced and labor charged for the particular installation
or repair. I agree you will not be
held responsible for any loss or damage to the aircraft, equipment or
articles left in case of fire, theft, and any other cause beyond your
control. If it becomes necessary for
you to employ a collection agency and or attorney to collect any billing for
this work, I agree to pay all collection costs, court costs, interest and
reasonable attorney fees and / or collection agency fees and hereby waive all
rights to claim exemption under any state or federal law. I agree to pay cash when work is
completed, or satisfactory terms to you; and until paid in full an express
mechanics lien is hereby acknowledged on this aircraft and /or equipment to
secure the amount charged for this work.

IMPORTANT WARRANTY INFORMATION
All Avionic repairs warranted
for 30 days. Discrepancies must be
reported within this period. Any billing adjustments must be made within 60
days for Spirit Avionics Ltd. to assume liability.

Payment is due upon completion, unless other arrangements
are made in advance. A finance charge
of 1.75% per month will accrue on all payments 30 days past due. Buyer agrees to pay all costs of
collection, including, including attorneys fees and court costs, in the event
of nonpayment according to the terms stated herein. All credit card fees will
be paid by the customer.
